  • Full Name
  • Title
  • Institution
  • A 30-50 word description/quote addressing one or more of the following topics:
  • Favorite/most rewarding aspect of CASE District II involvement
  • Successful outcomes due to something you learned at a CASE conference
  • Example of how CASE II has helped you by furthering the profession, establishing leadership or leading change in advancement
  • A photo of yourself (Images should be 72dpi/ppi and at least 200px wide.) - required
